MANDATORY SPORTS PHYSICALS 2012-2013 Sports physicals are mandatory for all student athletes who participate in an LLT afters school sport. A student will only need one sports physical per school year no matter how many sports they play for LLT. A sports physical was given to you in your registration packet. If you need another one you can print one off the LLT website www.lltacademy.com, it will be under middle school resources. The form will need to be filled out by your student's doctor and handed in to Coach Clemmer.
